Employee Benefits

Crown Point Health Suites offers benefits to Full-Time Employees working 32 hours a week or more. Employees on a 12-hour 2/2/3 schedule are considered Full-Time Employees.  All employee benefits are just a summary and are described further in the Employee Handbook and in correspondence with employees.

Medical Insurance   For eligible staff, the company will pay a substantial portion of the premium.   The first day of the month following a full month of employment
Dental Insurance   For eligible staff. Employee funded   The first day of the month following a full month of employment
Retirement Savings Plan   The Company will contribute 3% annually to the plan.   After one year of employment
Life Insurance   For eligible staff, the company will provide a $25,000 group life and accidental death and dismemberment.   The first day of the month following a full month of employment
Paid Time Off & Sick Leave   Company paid based on an accrual rate per years of service.   Upon hire
Holidays   Company paid for New Year’s Day, Memorial Day, July 4th, Labor Day, Thanksgiving Day and Christmas Day.   After 90 days of employment for eligible staff
Social Security   Company and employee contributions.   Upon hire
Employee Disability Benefit Plan   In the event of an injury on the job and proper actions have been taken, the company will assume responsibly for the necessary treatment as directed by a physician.   Upon hire
Short-Term Disability   Employee pays premium.   The first day of the month following a full month of employment
    Unpaid leave for certain reasons for up to 12 weeks per rolling 12-month period. Up to 26 weeks under certain conditions for families of military active duty.   Employees are eligible to take FMLA leave if they have worked for the Company for at least 12 months, and have worked for at least 1,250 hours over the previous 12 months.
Military Leave & Military Reserve   Unpaid leave while on active duty or while attending reserve camp.   Employees are eligible for Military Leave and Military Reserve if they have worked for the Company for at least 12 months, and have worked for at least 1,250 hours over the previous 12 months.
Jury Duty   With documentation, company paid for scheduled work hours missed while on jury duty   Upon hire
Funeral Leave   Unpaid leave granted from the date of death through the date of funeral without penalty to attendance.   Upon hire
Worker’s Compensation   The Company does not provide worker’s compensation insurance. The Company will pay for legitimate medical claims for employees who participate in the
Employer Disability Benefit Plan.
